Athletic Clearance Forms
Required form: Preparticipation Physical Examination / Must be completed by a physician.
Dear parents and guardians,
This year we have replaced Rank One with Healthy Roster. Successful completion of Healthy
Roster allows your athlete to participate in their sport. This is completed annually for ALL
athletes in the APS district.
If you do not have a Healthy Roster profile for your child, follow these steps:
- Fill out this google form with your contact information

- Make sure you use a parent email address and not a student.
- Wait for an invitation email from Healthy Roster. It is not automatic, the Athletic Trainers
have to manually invite you. Please be sure all information is correct, especially the
parent email you use.
- Once you’ve been invited and your Healthy Roster profile has been successfully created,
you can now work on the forms necessary for your child to participate in athletics.
Tips:
- The names used on each form you upload MUST MATCH the name on your athlete’s
Healthy Roster profile.
- Please take CLEAR pictures of all paperwork that you upload and make sure you
capture the entire page in the picture.
Medical Eligibility Form (Physical Exam) (requires picture)
- Physical exam must be dated and performed by a physician, PA, or NP after April 1, 2023.
- The page with the practitioners signature and box checked clearing for all sports
participation is the page that should be uploaded.
NFHS Concussion Certificate (requires picture)
- Upload the certificate earned after completing the concussion course on

Insurance Card (requires picture)
- Upload a picture of the front and back of your athlete’s medical insurance card.
- If you have TRICARE, upload a photo of a piece of paper that says “Tricare”. You do not
need to upload your military ID.
The following forms are completed directly through Healthy Roster and do not require a photo
upload of any documents:
- APS Athletics Parent Acknowledgement Form
- NMAA Consent to Treat
- Concussion Education and Acknowledgment
- APS Extracurricular Code of Conduct
- APS District Voluntary Activities Participation and Acknowledgement and Assumption of
Potential Risk Form
Once all of this has been submitted, we will work as a team to go through and manually approve
all paperwork. We generally are able to have things approved within 3 business days contingent
upon athletics schedules and coverage. We look forward to seeing your athlete participate!
